Abstract. Several computer-based approaches to Clinical Guidelines have been developed in the last two decades. However, only recently the community has started to cope with the fact that Clinical Guidelines are just a part of the medical knowledge that physicians have to take into account when treating patients. The procedural knowledge in the guidelines have to be complemented by additional declarative medical knowledge. In this paper, we analyse such an interaction, by studying the conformance problem, defined as evaluating the adherence of a set of performed clinical actions w.r.t. the behaviour recommended by the guideline and by the medical knowledge.

Abstract. Clinical Guidelines (CGs) capture medical evidence, but are not meant to deal with single patients ’ peculiarities and specific context limitations and/or constraints. In practice, the physician has to exploit basic medical knowledge (BMK) in order to adapt the general CG to the specific case at hand. The interplay between CG knowledge and BMK can be very complex. In this paper, we explore such interaction from the viewpoint of the conformance problem, intended as the adherence of an observed CG execution trace to both types of knowledge. We propose an approach based on the GLARE language to represent CGs, and on an homogeneous formalization of both CGs and BMK using Event Cal-culus (EC) and its Prolog-based implementation REC, focusing on “a posteriori ” conformance evaluation.

Abstract. Medical guidelines and protocols are documents aimed at improving the quality of medical care by offering support in medical decision making in the form of management recommendations based on scientific evidence. Whereas medical guidelines are intended for nation-wide use, and thus omit medical management details that may differ among hospitals, medical protocols are aimed at local use within hospitals and, therefore, include detailed information. Although a medical guideline and protocol concerning the management of a particular disorder are related to each other, one question is to what extent they are different. Formal methods are applied to shed light on this issue. A Dutch medical guideline regarding the treatment of breast cancer, and a Dutch protocol based on it, are taken as an example. 1

Abstract. Medical protocols and guidelines can be looked upon as con-current programs, where the patients dynamically change over time. Methods based on verification and model-checking developed in the past have been shown to offer insight into their correctness by adopting a logical point of view. However, there is uncertainty involved both in the management of the disease and the way the disease will develop, and, therefore, a probabilistic view on medical protocols seems more appropri-ate. On the other hand, representations using Bayesian networks usually involve a single patient group and do not capture the dynamic nature of care. In this paper, we propose a new method inspired by automata learn-ing to represent and identify patient groups for obtaining insight into the care that patients have received. We evaluate this approach using data obtained from general practitioners and identify significant differences in patients who were diagnosed with a transient ischemic attack (TIA). Finally, we discuss the implications of such a computational method for the analysis of medical protocols. Abstract—The transformation of a document-based medical guideline into a computer-based decision support is a time-consuming and error-prone activity. One way to alleviate this burden is by facilitating, as much as possible, the (semi)automatic implementation and further validation of the knowledge intensive tasks embedded within medical guidelines. This paper presents a bilingual (English and Spanish) “proof of concept ” simulation framework and computational test-bed, called V.A.F. Framework, that takes advantages of both CommonKADS methodology and Semantic Web technologies (OWL, SWRL, and OWL-S) to enable experiments (simulations of clinical situations) that allow overcoming the main barriers to successfully express medical guidelines in an executable form compatible with Electronic Medical Records (EMRs).To demonstrate how higher integration between EMRs and evidence-based medicine can be accomplished, this paper focuses on the “Acute Red Eye”, a clinical ophthalmologic domain known by General Practitioners (GPs) that usually requires the intervention of ophthalmologists (specialised physicians), so medical referral guidelines as well as ophthalmology medical guidelines need to be codified and integrated with EMRs.
