This activity has been designed to meet the educational needs of physicians, nurses, and pharmacists involved in the management of hospitalized patients with diabetes. Statement of need/program overview Hospitalized patients are an appropriate group to receive screening, diagnosis, and counseling about diabetes. In 2002, diabetes was the third most common comorbidity recorded on hospitalized discharge records, present in 11.8 % of patients; in 1997, diabetes was ranked as the fourth most common morbidity, with 9.5 % of patients affected. Moreover, because hyperglycemia is frequently missed in hospitalized patients, the true prevalence of diabetes in this population is more likely greater than 17%. These statistics reflect the steady growth of diabetes as a public health problem in the United States. Poor outcomes and morbidities such as stroke, nosocomial bacteremia, limb amputation, and acute myocardial infarction have been linked to hyperglycemia in hospitalized patients. Improving glycemic control in the inpatient setting will have positive implications for both short-term and long-term outcomes in patients, regardless of whether diabetes has been diagnosed in these patients. Proactive assessment of an inpatient’s

To understand how best to enable researchers ' exploitation of the potential for discovery of information by combining information from multiple, diverse and distributed data resources. o What strategies and methods would we advise them to use? o What tools will best support their work? o What cooperative plans can we make for developing the strategies and tools and for sharing the resources that they require? To investigate the strategies supported by existing tools. o Will scientific workflows and data federation remain separate lines of development and independent tools that researchers may use? Or can we see synergies that should be supported? o What are the methods that best work today? o What are the tools currently available? o How do they compare from the researcher's point of view? o What are their deficiencies and how should they be addressed? To understand the similarities and differences facing different scientific disciplines exploiting diverse distributed data sets. o Are there fundamentally different requirements for each discipline? o Is it appropriate to organise by discipline in order to reduce data exploitation costs within that discipline? Or will that generate discipline silos that inhibit interdisciplinary investigations? Chronology of Events: This sections details the meetings held during the theme, external meetings attended and talks given not detailed in the research outputs section.
